Heavyset Tech now provides a flexible way to add multiple disclaimers and opt-in checkboxes to your appointment form. This feature allows you to present important information to customers and optionally require acknowledgement before submitting the form.
This article walks you through how to create and manage disclaimers, enable opt-in checkboxes, and control form-submission requirements.
Accessing the Appointment Page Settings
Navigate to:
https://admin.heavyset.tech/settings/appointment-page/style/editUnder the Form Text section, click the Edit icon.
Adding and Editing Disclaimers
Inside the Form Text editor, you’ll see two initial disclaimer fields:
Disclaimer 1
Disclaimer 2
These text boxes are optional—add content only if needed.
Dynamic Disclaimer Expansion
If you enter text into Disclaimer 2, a new field—Disclaimer 3—will automatically appear.
This behavior continues as you add content, allowing you to create as many disclaimers as needed.
Supported Formatting in Disclaimer Text
Disclaimer fields support enhanced formatting with the following syntax:
Links
([[url,Title,https://domain.com/path/]])
This renders a clickable hyperlink.
Line Breaks
\n
Creates a new line.
Bold Text
[[b,Your Text Here]]
Use these formatting options to improve clarity and usability for end-users.
Saving Your Changes
When you’re finished editing your disclaimers:
Click Okay
Then click Submit to save your changes
Use Preview to confirm how it will look on the customer-facing form
Configuring Opt-In Checkboxes
Below the Form Text section, you’ll find configuration options for opt-in checkboxes.
You’ll see:
Opt-In Checkbox 1 (default: disabled)
A corresponding Required 1 toggle
Dynamic Checkbox Expansion
When you enable Opt-In Checkbox 1, a new option appears:
Opt-In Checkbox 2, also defaulted to disabled
…and so on as you enable additional checkboxes.
Each enabled opt-in checkbox will appear on the appointment form, tied to the corresponding disclaimer text in sequential order.
Setting Required Checkboxes
Next to each Opt-In Checkbox field is a toggle labeled Required X.
When enabled, the user must check the associated checkbox before submitting the appointment form.
If left disabled, the checkbox remains optional.
This is useful for legally required disclaimers or mandatory acknowledgements.
Saving Checkbox Settings
Just like with disclaimers:
Click Submit to save your checkbox configuration
Click Preview to verify that the checkboxes display properly on the form
