Overview
Appointment forms support disclaimer messages displayed near the email/submit section. Each disclaimer can optionally include a checkbox that must be checked before submission.
Steps
- Log in to the HeavySet Tech Dashboard
- Navigate to:
Style → Edit Style - Select the Appointment Form to edit
- Under Form Text click the Edit page text button
- Scroll to the Disclaimer section
- Edit the desired field
- Click Save
Formatting
Disclaimer fields support the following syntax:

Checkbox Behavior
Each disclaimer supports an optional checkbox:
- Active — displays a checkbox
- Required — blocks submission unless checked
- Inactive — no checkbox displayed
If a required checkbox is not checked, the form shows:
"Please accept all required terms"
Positioning
Disclaimers can be displayed above or below the submit button using the Disclaimer First Button setting.
Notes
- Submission audits store the disclaimer content at the time of submission
- If only one disclaimer field is visible, the account may be using the legacy single-disclaimer configuration
